I really think thats the best thing you can do. Sometimes depression feels too tough to handle without meds or some sort of professional help. Simply going in and saying "I think I have depression and its affecting the way I'm living." is just fine and right to the point. Explain to her that you'd like to get your life back on track, your mom is not helping you because she thinks you're just being lazy, and that you need help. If you feel like she wont take action, tell her you've had suicidal thoughts. If you share that with a counceler they are obligated BY LAW to do something about it.
But I dont think it'll come to that. I think just talking about how you feel (you have to be honest even if you're embarassed) will make him/her see whats going on.
