Doctor/Blood Test

I need a shot, and a blood test. I want to get the blood test done at the hospital, which says I need an order from a physician. Well, it wasn't my physician that ordered it, it was my psychiatrist, and she's too far away to just walk over and get a note from her.

Can I walk in and ask the hospital what I need to do on a Sunday for that? Or do I have to wait until Monday? And where would I even go in and ask? And do I need a parents permission, I'd rather go by myself. [I'm 15, parents would give money, and I have insurance]

Then, to get the vaccination I need, can I just make an appointment by myself and walk in? Again, I'd like to go alone, but I don't know if I'm allowed to go without a parent? And I kind of have a fear of calling places, so I'd rather just call to make the appointment >.< Not to ask other questions.
