Pissing me off - how to apologize?

According to the therapists, I have Sadistic Personality Disorder (now put under Antisocial Personality Disorder). I had a therapist before but she pissed me off to no end. I have another one but now she is on a vacation. I've asked other people this and I'm still not understanding it. So, how do you apologize, as in, how do you say sorry and when? If and when I do, others say it's at the wrong times.

I've asked some people and it makes not much sense. What does saying sorry mean? When and how do I say it?

Well you gotta try to say it like you mean it. But i'm thinking that could be quite hard because of your disorder? Just try not to say it in a way that it is obvious that you don't even mean it or it is forced.

When to say it? Say it when you have done something that is not nice or if someone is telling you about a bad experiece or something like that.

Hmm... so if I do something to physically injure them or burn something, then I say sorry?
How do I say it so it's not obvious or forced? I don't get that.
