If its an external exam and you don't want to strip, the doctor has a protocol to not make you to. If it is necessary to take a pap smear test, only then you have to strip. As for doctor knowing whether you masturbate, normally there is no way to tell from body signs so no worries here. As a doctor and a gynae, they don't care whether you masturbate or not, they are not American high school kids who get worked up over it.

If she is sexually active, the doctor will be able to tell easily. But not if she masturbates, both aren't generally the same thing. But a doctor would find it more normal if a girl masturbates, which means she is sexually healthy.
