I would consider it too young to have sex though, I know that I could not've had sex at 12 years old, that'd have been totally ridiculous. Then again, it really is a matter of what she really wanted.

Anyways, you really need to try and talk to her about not doing so again. 12 is way too young to have sex. What would she do if she got pregnant or contracted an STD?

I would just talk to her about it and try to get her to understand that she is too young to do it, that it has major repercussions. I don't know what it is, but it seems like the kids nowadays are getting into sex early. I always hear about middle school girls giving handjobs and blowjobs and they're all called sluts and skanks... And she probably doesn't want that to happen to her. Just sit down and tell her it's in her best interest to wait at least a few years before having sex.

And if that doesn't work... Well... I suppose you should take it to her mom, but I'd only do it if her mom is fairly cool about things. Just sit her down and tell her you want to tell her something about her daughter so she can help her out, and that you're concerned for her well-being yadda yadda... Although I warn you, she'll probably hate you for it but it'd be in her best interest if there is no other way.
